Abstract

Introduction: From the age of 75, frailty is a common geriatric condition. Frailty often has a negative effect on oral health. Four years ago the CTM in Groningen started providing oral care to frail elderly in the CvG. This study contributes to the inventory of the dental care demand and the possibilities of access to oral care of frail and non-frail elderly in the Groningen region, so that the CvG can respond to this in the future. The research question is: ‘What is the difference in dental care demand and possibilities of access to oral care between frail and non-frail elderly (75+) in the Groningen region?’. Materials and Methods: A cross-sectional questionnaire study was conducted among elderly (75+) who speak the Dutch language and who live in the Groningen region. The questionnaire was distributed at various locations in the Groningen region and collected data for the independent variable ‘frailty’ and the two dependent variables ‘dental care demand’ and ‘possibilities of access to oral care’. The independent variable ‘frailty’ was assessed using the GFI. The dependent variable ‘dental care demand’ was assessed on ten aspects and the dependent variable ‘possibilities of access to oral care’ was measured using seven aspects from the questionnaire. In SPSS a Chi-square test or a Mann-Whitney U test was used to analyze the differences in care demand and accessibility between frail and non-frail elderly. Results: 129 elderly with a median age of 83 years [IQR 78-87] participated in this study. Out of the total sample, 40% (n = 52) could be identified as frail. The majority of frail and non-frail elderly had their own dentition. Out of the total sample, 71% (n = 92) indicated that they visit the dentist every (half a) year for a check-up. There was no statistically significant difference between frail and non-frail elderly on any aspect of the dental care demand. Out of the total sample, 13% (n = 17) indicated that the dental practice is difficult to reach and 8% (n = 10) indicated that the dental practice is not easily accessible. The possibilities of access to oral care only differed statistically significantly between frail and non-frail elderly on the aspects 'current living situation' and 'being able to go to the dental practice independently': non-frail elderly more often lived completely independently (p < .001) and more often indicated that they could go to the dental practice independently (p < .001). Conclusion: Frail and non-frail elderly have a dental care demand that did not appear to differ statistically significantly between the two groups of elderly. Both frail and non-frail elderly indicated that professional dental care is accessible to them. However, the possibilities of access to oral care of frail elderly appeared to be less, because they were statistically significantly more likely to not live independently and more often indicated that they needed help from others to get to the dental practice.
