Introduction The plant-based diet and consuming less animal products is becoming increasingly popular, but little is known about this diet in relation to periodontal health. The research question is 'What is the association between a more or less plant-based diet and periodontal health?'. The aim is to make this association more clear, so a more targeted and feasible advice about dietary pattern and adjusting it can be drawn up. Method In this cross-sectional study, the database of the National Health And Nutrition Examination Survey (NHANES) 2003-2004 was consulted. Research data from a nutritional questionnaire and a periodontal examination of approximately 10,000 people were assessed, 2141 were included. Periodontal health was assessed based on measured pocket depth (PD), attachment loss (AL), and bleeding index (BoP). The degree of plant-based diet was shown in 3 different indices: plant-based diet index (PDI) in which a positive score was assigned to plant food and an inverse score to animal foods. For healthy PDI (hPDI) a positive score was only assigned to healthy plant foods (vegetables, fruit, legumes, whole grains) and for unhealthy PDI (uPDI) only a positive score was assigned to less healthy plant foods (processed products). A logistic regression analysis was used, adjusting for socio-demographic, lifestyle, and medical confounding variables. Results There is no significant association between PDI and periodontal health. For hPDI, a significant association with periodontal health was found, for the raw and the adjusted analysis (p < ,01). For uPDI, a significant association with periodontitis was found, only observed in the raw analysis (p < ,05). Discussion A more healthy plant-based diet has a significant association with periodontal health. There is a possible link between an unhealthy plant-based diet and an unhealthy periodontium. The findings should be interpreted with caution. Follow-up research is necessary to make reliable statements about step-by-step dietary adjustments.
