Abstract

Introduction: Fear of the dentist is a common phenomenon. 40% of the Dutch adults are anxious and 12% are very anxious to visit a dentist. Patients with extreme dental anxiety, are treated in centres for special dentistry ‘Centrum Bijzondere Tandheelkunde (CBT)). In these centres, various treatment methods can be used to treat patients with dental anxiety. Besides specific dental phobia, other psychological disorders also occur in Dutch adults. When patients have several psychological disorders at the same time, this is also known as psychological comorbidity. Dental anxiety has also been associated with psychological disorders and especially with mood, anxiety and substance disorders. Within dentistry, almost no research has been done on this. This study therefore reports the prevalence of psychological comorbidity in patients with extreme dental anxiety. Furthermore, it investigates to what extent patients with psychological comorbidity are on average more anxious than patients from the group without comorbidity. In addition, we investigated whether there is an association between treatment approach and psychological comorbidity. The aim of this study was to gain understanding of any comorbidity present in patients referred to CBT because of extreme anxiety for the dentist in addition, to understand whether there was a correlation between treatment approach and comorbidity. Material and method: This study was a retrospective study of competent adult patients referred to CBT due to extreme dental anxiety. Data were collected from patient records. Data from 89 patients were included for this study. Prevalence of psychological comorbidities in patients with extreme dental anxiety were identified. Four questionnaires were used to estimate dental anxiety, the DAS, K-ATB, DPFR and AS. It was investigated whether there was a relationship between anxiety scores and four categories of psychological comorbidities. Psychological comorbidities included mood, anxiety, substance disorders and other disorders. This was tested with the independent t-test and Mann-Whitney U test. Fisher's exact test was also used to test whether there was consistency between treatment approach and psychological comorbidities. Results: In this study, 77.5% (n=69) of patients had psychological comorbidity. No significant difference was found in any of the anxiety scores between patients with or without psychological comorbidity. There was also no significant difference between mood, anxiety and substance disorders and other disorders and anxiety scores. Moreover, there was no significant association between treatment approach and psychological comorbidity. Conclusion: In this study, no significant difference was observed between anxiety scores in patients with and without comorbidity. There was also no significant difference between mood, anxiety and substance disorders, and other disorders and anxiety scores. Moreover, there was no significant relationship between treatment approach and psychological comorbidity. The study results may be explained by the fact that the study population was too small. It is recommended that this study be continued with a larger study population.
