Abstract

Background: The absence of teeth in the esthetic zone often results in a social- and emotional burden. Due to its good functional and esthetic proportions, resin bonded fixed dental prostheses (RBFDP) are frequently used when missing anterior teeth. These RBFDP are recommended when aiming for the optimal preservation of dental tissue and keeping costs at a minimum. Over the past few years lithium disilicate resin bonded fixed dental prostheses (LDACE) have been manufactured and placed in cases of missing teeth in the anterior region, due to its good esthetic properties. The aim of this study is to find out what the incidence rate of mutations to the LDACE restorations is over time. In addition, this study investigated of the incidence rate of changes is dependent on the practitioner who placed the LDACE. Materials and methods: In this retrospective study, a total of 108 LDACE restorations were assessed by examining them on the twelve criteria of the “modified USPHS” assessment form. The LDACE have been placed three different practitioners, by which they were divided in three groups. The incidence rate has been determined for each criterion. Subsequently, the difference in incidence rate between the groups was determined per criterion in order to investigate if any significant differences between the groups could be demonstrated. Results: The 108 LDACE have an average of 32,47 months at risk. 0 Criteria were assessed as a failure during the assessments. Also, no carious lesions or fractures of the LDACE were found within this study. The most assessed changes of the LDACE were mild inflammatory changes of the surrounding soft tissue within the criterium ‘Gingival Health’ and slight discoloration within the criterium ‘Color Match’. The mild inflammatory changes were assessed at 22 LDACE, with an annual incidence rate of 0,07415. The slight discolorations were assessed at 17 LDACE, with an annual incidence rate of 0,05730. Significant differences were found between at least two different practitioners and practices within the criteria ‘Adaptation’, ‘Color Match’, ‘Marginal Adaptation’, ‘Polishability’, ‘Surface Staining’, ‘Gingival Health’ and ‘Wear Antagonist’. Conclusion: In this study it has been shown that there have been few changes in the LDACE, but no failures. Therefor it has shown that LDACE are a good treatment option in the absence or replacement of anterior teeth. Significant differences were found in the incidence rate of changes in LDACE from different practices and practitioners. More research to further investigate these differences is recommended.
