Abstract

Objectives: (1) To examine differences in gait features between patients with chronic low back pain (CLBP) and pain-free controls during a 6-minutes walking test (6MWT), (2) to establish the relation between the progression of gait features and pain intensity during a 6MWT in patients with CLBP. Design: Exploratory cross-sectional. Methods: Eight patients with CLBP (5 male; mean age, 50.8 years) and eight pain-free controls (5 male; mean age, 48.4 years) participated in this study. Gait characteristics (n=19) derived from trunk accelerations were used to quantify the time-dependent variations and dynamics of walking. For the CLBP group, pain intensity was verbally monitored each minute with a 0-10 pain rating scale. Differences in gait features between both groups were assessed with a Principal Component Analysis (PCA). Associations between discriminatory gait features and pain intensity changes over time were assessed with Spearman’s correlations. Results: Patients with CLBP exhibited higher Harmonic Ratios and lower amplitudes of trunk accelerations compared to controls. Pain intensity increased 1.2 points in patients with CLBP during the 6MWT. Increased pain intensity was correlated with increased stride time variability (rs=0.80) during the 6MWT, but not with the other parameters. Conclusion: Patients with CLBP revealed higher gait symmetry and slower pace compared to controls, albeit no differences in variability, regularity, smoothness, stability, and predictability became manifest during walking at a self-selected speed. The progression of the discriminatory gait features during the 6MWT were not substantially related to increased pain intensity.
