Abstract

Study design: Cross sectional study. Objective: The study-aim was to evaluate kinetic and spatiotemporal wheelchair propulsion technique parameters among individuals with spinal cord injury (SCI) and shoulder pain and those without shoulder pain. Setting: Multiple SCI-specialised rehabilitation centres in the Netherlands. Methods: Thirty-eight experienced wheelchair users with SCI (aged 47.9 (12.4) years, time since injury of 14.7 (9.6) years, 18.4% tetraplegic and 65.8% with a complete lesion) participated in this study. They were divided in a ‘shoulder pain’ and ‘no-shoulder pain’ group using the Local Musculoskeletal Discomfort scale. The last minute of collected force and torque data onto an instrumented handrim during two 3-minute blocks of submaximal manual wheelchair propulsion on a motor driven treadmill were analysed. Results: There was no significant difference between the pain and no-pain group in power output and propulsion velocity. After correction for statistical defined confounders, body height and time since injury, linear regression analyses showed a 0.3s (95% CI 0.50 – 0.09) shorter cycle time, from the start of a push to the next, a 15.5° (95% CI 27.4 – 3.7) smaller rim contact angle and 7.8% (95% CI 15.3 - 0.3) less variability in the work per push in the ‘shoulder pain’ group. Conclusion: Individuals with shoulder pain in this study propelled with smaller, shorter pushes and had a decrease in the variability of the work per cycle, compared to manual wheelchair users without shoulder pain. This association between shoulder pain and handrim propulsion and its causal direction warrants further investigation.
