Abstract

People with cervical dystonia (CD) experience excessive, sustained muscle contractions in their neck which causes involuntary movements and abnormal postures in the neck and head. CD limits the movements of the neck and this may negatively affect head movements during driving, especially before crossing an intersection. The aim of this study was to establish the differences in head movements before crossing an intersection between CD patients and healthy controls in a driving simulator and the possible compensation strategies of CD patients. The head movements of patients with CD (n = 10) and healthy controls (n = 6) were measured while they performed a comparable driving task with multiple intersections in a driving simulator. The range of motion in the neck (Range), the number of head turns (Number of Turns) and the time before the first head movement (Time First Peak) at 100 m ahead of the intersection until the end of the intersection were analyzed with Mann-Whitney U tests. In addition to these outcome measures, the time they took to drive 100 m to the intersection (Approaching Time) was also analyzed with a Mann-Whitney U test. The results showed differences between CD patients and healthy controls in Range and Number of Turns on two intersections. However, these differences were not significant (p > 0.05). This can be due the small sample size of the control group. In conclusion, we did find differences in head movements during driving between CD patients and controls, but further research is needed before concrete conclusions can be made about their head movements during driving.
