Abstract

Abstract: During dialysis the physical fitness of renal patients becomes extremely impaired. After transplantation it remains lower compared to healthy controls. The impaired physical fitness also influences physical activity (PA) and health-related quality of life (HRQOL). This study aims to improve (1) physical fitness, (2) PA and (3) HRQOL of renal transplant recipients (RTR) through a 12-week exercise intervention. Methods: 221 RTR were randomly assigned to one of the three study arms: 1) Exercise group (EG), 2) Exercise and dietary group (EDG) or Control group (CG). The EG and EDG both received 12-weeks of aerobic and strength training and sports and play two times a week. Furthermore, the EDG received dietary counseling. The CG received usual care. Physical fitness outcomes were: cardiorespiratory fitness and power output (Wpeak) at the lactate threshold, aerobic threshold and during submaximal exercise and muscular strength. PA outcomes was measured as daily steps and with the Baecke questionnaire. HRQOL was determined using the SF-36. Results: The results show that physical fitness has improved after training, especially muscular strength. The intervention groups improved significantly more compared to the CG on Wpeak (p=.001), 50% Wpeak (p=.029) and muscular strength (p<.050). Furthermore, mental health (p=.023) of the SF-36 improved significantly in the intervention groups. Conclusion: This study shows that physical fitness of RTR, power output, efficiency and muscular strength, improves after training. The training also influences HRQOL positively. Further research is needed to determine long-term effects of the intervention and the 15-month results of lifestyle coaching and dietary counseling.
