Objective: Metformin is the medicine of first choice in the treatment of type 2 diabetes mellitus (T2DM). Up to 30% of patients using this drug suffer from a decreased resorption of vitamin B12. This study aims to describe the prevalence of vitamin B12 deficiency in patients with T2DM taking metformin compared with patients with T2DM without metformin use. Research design and methods: This study had a cross-sectional design. All patients visiting the diabetic outpatient clinic of the Isala clinics in Zwolle between May 1st and July 27th 2012 were asked for participation. Participation included a venipuncture with measurement of Hb, MCV and vitamin B12 levels. Vitamin B12 deficiency was defined as serum B12 concentrations <150 pmol/L. Results: Of all T2DM patients (n=298) the prevalence of vitamin B12 deficiency was 9,7% (95% CI 6,6% to 13,7%). Of the T2DM patients not taking metformin, the prevalence was 4,4% (95% CI 1,6% to 9,4%) compared with 14,1% (95% CI 9,2% to 20,4%) in those who did use metformin (P 0,006). Metformin use could not predict the chance of having anemia or neuropathy. Conclusions: Metformin use gives a higher prevalence of vitamin B12 deficiency. However, metformine use cannot predict the chance of having anemia or neuropathy.
