Abstract

Background: The development of cardiac disease, as well as the prognosis after cardiac surgery is influenced by an unhealthy lifestyle. During the waiting period for surgery and hospitalization cardiac patients experience declines of physical, social and psychological functioning. A program supporting patients during the waiting period prior to and directly after hospitalization is suggested to better prepare patients for surgery and to promote a healthy lifestyle, which possibly speeds up recovery from surgery and prevents further progression of cardiovascular disease. Objective: The purpose of the current study was to determine the feasibility, defined in terms of the accessibility, compliance and training load, of the pre and postoperative (PRE+POST) rehabilitation program for cardiac surgery patients. Study design: Retrospective observational study. Study population: 41 Patients scheduled for coronary artery bypass graft surgery, valve surgery, aortic surgery or combinations of these surgeries in the University Medical Centre Groningen (UMCG), the Netherlands between March 2015 and January 2016, participated in the PRE+POST rehabilitation program and were analyzed in the current study. Intervention: During the waiting period prior to surgery patients received multidisciplinary rehabilitation including bicycle and strength training for approximately four weeks, three times a week (PRE phase). Directly after hospitalization the program was continued with a postoperative inpatient phase of three weeks (POSTin phase, daily bicycle and strength training, starting when patients’ physical condition after surgery was sufficient to perform the exercises), followed by a four week postoperative outpatient phase (POSTout phase, twice a week bicycle and strength training). Main study parameters: Outcome measures were the percentage of eligible patients following the PRE+POST rehabilitation program, the compliance to follow the sessions of bicycle and strength training and the training load during strength and bicycle training. Results: Of the eligible patients 64% followed the PRE+POST rehabilitation program. During the PRE and POSTout phase patients followed approximately 69% and 79% of expected sessions of strength and bicycle training respectively. Increases in training load of bicycle and strength training were seen between the sessions starting and ending each rehabilitation phase. Conclusion: The rehabilitation program showed an acceptable accessibility, compliance and training load. Therefore, the feasibility of the PRE+POST rehabilitation program was considered to be acceptable. Improvements to the program are required, namely regarding the bicycle test, adherence to the protocol, the compliance to follow the training sessions and the duration and intensity of the program. Furthermore, the analysis of feasibility should be improved by using the registering systems and the administration of adverse events.
