Abstract

Introduction: Parkinson’s disease (PD) is one of the most common progressive neurodegenerative disorders. Several rating scales were developed to outline PD problems, but none of them was focusing on bed mobility (BM) and sit-to-stand (STS) transfers in end-stage PD patients, Hoehn & Yahr (H&Y) stage IV & V. Therefore, the aim of this study was to develop an observation scale including BM and STS transfers, which was especially for end-stage PD patients in nursing homes. Method: Four end-stage PD patients were included in the study. The participants had distinct motor problems in the areas of BM and STS movements, but were to a certain extent capable of performing transfers partly independent. BM and STS transfers of the patients were recorded. Students and professionals filled out the Parkinson Transfers Observation Scale (PTOS) while watching a compilation of the video recordings. After conducting a pilot study, the definitive PTOS was tested on inter-rater and intra-rater reliability by using Cohen’s Kappa (K). Results: The overall inter-rater agreement of the PTOS amounted to a mean K of 0.32. K’s per patient and for each subcategory were also calculated and ranged between 0.27 – 0.42 resp. 0.02-0.47. The test-retest reliability of the PTOS had an overall kappa value of 0.50. The K values for the observers separately showed values of 0.46 and 0.52. All reliability tests showed insufficient K values for the PTOS to be reliable. Conclusion: The newly developed Parkinson Transfers Observation Scale, meant to be used for end-stage Parkinson patients in nursing homes, is not reliable yet. The scale needs important adjustments before it can be used in practice.
