The aim of the study was to investigate if a 16-week low-intensity hand rim wheelchair intervention will lead to changes in wheelchair specific fitness in deconditioned persons with a chronic spinal cord injury (SCI). For this randomized controlled trail twenty-six inactive persons with a chronic SCI were assigned to either an intervention or a control group. The intervention consisted out of a 16 week wheelchair exercise training twice a week at an intensity of 30-40% of the heart rate reserve (HRR). Results showed an increase in power output en velocity over the training sessions. But, did not show a positive effect on wheelchair-specific fitness except for the anaerobic power. The wheelchair specific fitness of this inactive group is very low compared to the general population with a SCI and it is difficult to improve the wheelchair specific fitness without high risk of overuse injuries. The cycle of deconditioning should be stopped before the wheelchair-specific fitness has decreased to a level this low.
