Abstract

Background: Eccentric strength training appears to be a more effective training program for chronic stroke patients than concentric strength training. However, the isokinetic exerciser used in interventions reduces the feasibility due to the costs and availability of the equipment. Objective: To establish the feasibility of an eccentric strength training program with dumbbells and rubber bands in chronic stroke patients. Methods: Four chronic stroke patients participated in this pilot study which was part of a larger research project. The participants received four weeks of eccentric strength training and four weeks of task-oriented strength training in a cross-over design. The outcome measures were; feasibility, measured with the Intrinsic Motivation Inventory (IMI), the adherence and dropout rates; strength, measured with a hand-held dynamometer (HHD) and upper limb function, measured with the Action Research Arm Test (ARAT). Results: A total score of 82.9% was reported on the IMI. There were no dropouts and the adherence was 100%. Strength of the shoulder and elbow increased between 2.7N and 20.4N. Upper limb function increased with a score of 2.3 on the ARAT. No significant differences were found. Conclusion: Eccentric strength training with dumbbells and rubber bands is feasible and has a positive trend of benefit as measured with the IMI, the absence of dropouts, the 100% adherence rate, the HHD and the ARAT. The training program is low-cost, requires little assistance from a therapist and would be suitable to implement in stroke rehabilitation. Future research with more participants is necessary to support the findings of this study. Key words: stroke, eccentric, strength training, feasibility, upper limb
