Abstract

Introduction: Neural inertia is the resistance of the central nervous system to changes in the current state of consciousness. This phenomenon manifests as a higher effectsite concentration required for induction than the effectsite concentration when consciousness is restored. Research on neural inertie in humans is yet to be conclusive. In this thesis, neural inertia is studies in anesthesia induced by the novel hypnotic remimazolam. Methods: twelve healthy volunteers received during three sessions different concentrations of remimazolam according a step up-step down regime, with or without remifentanil. Based on the Bispectral Index (BIS), the concentration responsible for 50% of the effect (C50) was determined for the induction and emergence phase. Using a sigmoid model, the BIS scores were predicted and the relationship between effectsite concentration and BIS was studied. Explanatory factors for the variability in C50 were sought. Results: 93% of the BIS-scores were higher during the emergence than induction at the same effectsite concentration. The C50 during induction was significant lower than the C50 during emergence in anesthesia of remimazolam alone. No significant difference between men and women were found. Conclusion: in contrast to previous studies with propofol, anesthesia induced by remimazolam showed no neural inertia. The effect site concentration during induction was lower than during awakening. The influence of remifentanil is not yet known.
