Background: In the Western world, breast cancer is the most common cancer among women. Screening for breast cancer can help detect cancer at an earlier stage, which ultimately leads to a better prognosis and survival. The aim of this study is to measure the effect of the screening program introduced in Curaçao in 2010 on the distribution of the TNM-stage. Methods: A retrospective cohort study was performed using data collected before breast cancer screening (between January 2006 and December 2009) and during screening (between January 2015 and December 2018). Women were included if aged between 45 and 75 years and diagnosed with breast cancer or ductal carcinoma in situ (DCIS). Results: A total of 728 women were included, 283 were diagnosed with breast cancer or DCIS in the period before screening (mean age was 59.4 ± 8.6). During screening, 445 women were diagnosed with breast cancer or DCIS (mean age was 60.2 ±8.3). We found significant changes in the distribution of TNM stage (p<0.001). The number of detected DCIS increased approximately 10%. When you look more closely to various ways DCIS is detected (through the screening program or outside the screening program) in the period from 2015-2018, the percentage of DCIS in the screening population has doubled. Women with stage 4 breast cancer were also more often detected in the group that wasn’t detected by the screening program: an increase of 6%. Conclusion: The results show a significant difference in TNM staging in period before and during breast cancer screening in Curaçao. This difference shows an increase in low TNM stage and - remarkably enough - also an increase in TNM stage 4.
