No change in survival of children with Non-Rhabdomyosarcoma Soft-Tissue Sarcomas (NRSTS): a retrospective analysis from 1990 to 2018 in the Netherlands

Abstract

Background – Non-rhabdomyosarcoma soft tissue sarcomas (NRSTS) are very rare, especially during childhood. It is not clear yet if progress has been made in the survival of pediatric NRSTS. Until now, no national trend analysis on survival of pediatric NRSTS has been performed. Methods – NRSTS patients < 18 years, diagnosed between 1990 and 2018, were derived from the Netherlands Cancer Registry (NCR). Incidence rates were calculated for all pediatric NRSTS and trends were analyzed by using joinpoint analysis. Five-year Observed Survival (5Y-OS) was estimated by using the Kaplan-Meier method. Cox proportional hazard models were used to determine the effect of age, gender, stage at diagnosis and treatment on changes in survival over time. Results – In total, 466 children were included in this study. Incidence of pediatric NRSTS remained stable at an average incidence rate of 4.5 per million person-years. The 5Y-OS of patients with pediatric NRSTS was 75% in the period 1990-2018 and did not change over time. Children aged 0-2 years had the worst survival outcomes (5Y-OS of 65%), while 3-9 years old children had a 5Y-OS of 85%. Furthermore, patients who only received systemic therapy had the worst survival outcomes (5Y-OS of 19%), whereas patients who received locoregional treatment had a 5Y-OS of 90%. On multivariable analysis, sort of treatment did not influence the 5Y-OS over time. Conclusion – Survival of children with NRSTS in the Netherlands did not improve between 1990 and 2018. This problem might be addressed by establishing a better national and international collaboration between experts in pediatric and adult sarcomas.
