Abstract

Background: The benefit of post-operative radiotherapy (PORT) for high-risk cutaneous squamous cell carcinoma (CSCC) with tumor free margins after surgical excision is not clear. Guidelines recommend considering it, but no clear advice is given due to little evidence. Objective: The primary aim of this study is to compare the disease outcomes local recurrence (LR), nodal metastasis (NM), and distant metastasis (DM) for primary radically excised high-risk CSCCs that have additionally received PORT with similar primary radically excised high-risk CSCCs that have not received PORT. Materials and methods: Matched retrospective cohort study. Subjects were matched on gender, age, and whether or not they had perineural tumor invasion (PNI). Results: 43 tumors were included in the study, of which 14 CSCCs in the PORT group and 29 CSCCs in the surgery monotherapy group. No statistically significant results were found for disease outcomes. No LRs, NMs, or DMs were found in the PORT group. Disease progression was found for five CSCCs in the surgery monotherapy group. Conclusion: The benefit of PORT for high-risk CSCCs remains unclear. Though not statistically significant, the results did show a trend favoring the PORT group which is consistent with previous studies. Larger, multi-center, prospective studies should be performed to find out for which CSCC’s PORT is beneficial.
