Abstract

Background: Emergency medical services (EMS) providers provide pre-hospital care to people who need – often urgently – medical attention. Daily, EMS providers makes various decisions during a cognitive process called clinical reasoning. Clinical reasoning is seen as one of the core competencies EMS providers should learn during training. Aim: First, this research aims to gain insight into how EMS providers field of practice define ‘clinical reasoning in EMS’. Secondly, to identify the position and nature of clinical reasoning education during training for EMS providers. Methods: This study has a qualitative research design using a thematic analysis of semi structured interviews. Results: 19 participants were interviewed and after 16 interviews data saturation was achieved. Three themes emerged from the data: 1) clinical reasoning in EMS, 2) the position of clinical reasoning during training and 3) the content of clinical reasoning education during training Conclusion: Clinical reasoning in EMS can be defined as a cyclic process of flexible recurring and sometimes unconsciously conducted decision making. The determination of the main complaint(s) and formulating the working diagnosis with the corresponding treatment policy are seen as essential components. Within EMS training clinical reasoning is hardly positioned as a separate educational topic but is often seen as a part of other educational topics. Clinical reasoning education during training primarily involves learning to support the clinical reasoning process. The cognitive psychological aspects of clinical reasoning are less emphasized. A more detailed definition of clinical reasoning education during training and a clear alignment of EMS providers needs offer opportunities to improve education in clinical reasoning.
