Abstract

Post-transplant ureteral strictures can complicate the function of a renal allograft. Better risk stratification for this fibrotic complication is required. The hypothesis was that enhanced pro-fibrotic Transforming Growth Factor beta-1 (TGF-β1) production due to ischemic and ischemic-reperfusion injury in intra-transplant ureter tissue was associated with post-transplant ureteral stricture. Materials and methods: This retrospective case-control study (N=28) analysed TGF-β1 protein and gene expression in ureter biopsies taken at the end of cold ischemia time (T2) and after reperfusion (T3) using ELISA and qPCR. Controls were matched to cases on known risk factors for ureteral strictures including donor type, diabetes of the receiver, and gender, age, and body mass index (BMI) of both donor and receiver. Results: Intra-transplant TGF-β1 protein and gene expression in ureter tissue showed no significant correlation with ureteral stricture. Nevertheless, variation in TGF-β1 expression was observed. Significant correlations between TGF-β1 expression and donor characteristics included first deceased donor and prolonged cold ischemia time for absolute TGF-β1 protein concentrations at T2 and in change over time (T3-T2), second male gender of the donor for absolute T2 protein levels, and third lower BMI of the donor for absolute T2 gene expression. Discussion: Intra-transplant TGF-β1 expression in ureter tissue was not correlated with post-transplantation ureteral stricture in this study. Significant positive correlations of cold ischemia time and gender with TGF-β1 expression were found, which is comparable to literature on intra-transplant renal biopsies. Thus, future studies are required to determine whether early post-transplant TGF-β1 is correlated with ureteral stricture, and whether non-invasive ureter biopsies can replace invasive kidney biopsies for risk stratification for renal graft failure.
