Abstract

After an initial allergic reaction to a specific type of nut, a child often avoids all types of nuts because of the risk of a multiple nut allergy. The child is however usually tolerant to at least one other type of nut. To prove this, extensive, time-consuming diagnostics by means of oral food challenges are required. In addition, proving tolerance does not always lead to the (re)introduction of this nut into the diet. Strict avoidance of allergens, on the other hand, also negatively affects quality of life. The aim of this study is to make a recommendation on the diagnostics for allergies to other nut types in a child with a specific nut allergy. This is a retrospective study in which all children who received an oral food challenge in the Kinder Allergie Behandel Centrum of the Deventer Ziekenhuis from 2015 to 2018 were included. Information on sensitization and oral food challenges were collected from the patient files, after which it was determined whether there was an allergy, a possible allergy or tolerance to all different types of nuts. Information about diet at the time of referral and after diagnosis was collected. A total of 173 patients were included. A generalized nut allergy did not occur and tolerance to two types of nuts occurred in 95% of the children. Cashew nut allergy was the most common nut allergy (42%) and almond allergy was found to be rare (1%). After excluding a specific nut allergy, this type of nut was no longer avoided in 87-98% of the cases. This research shows that children are always tolerant to at least one nut species and that a less restrictive diet is fed after diagnosis. Extensive diagnostics of allergies to other nut types in children with a specific nut allergy is therefore useful.
