Introduction: A common problem with edentulous patients is that the denture no longer has enough stability or retention. As a result, these patients experience problems with chewing, speaking and the comfort is not optimal. An implant supported overdenture is a good solution for these problems. Objective: To make a comparison between a maxillary overdenture supported by four and six implants in the anterior region after an observation period of 10 years. Research questions: What is the survival rate of the implants placed 10 years ago in the anterior region of the upper jaw in both the group with four and the group with six implants? How is the patient satisfaction with regard to function, aesthetics and chewing ability? What are the differences in satisfaction between the group with four and the group with six implants? What are the differences in bone level between the group with four and the group with six implants? What is the difference in bone level compared to the 1- and 5-year results? What technical and biological complications have occurred? Material and methods: Ten years ago, 50 patients who had problems with their maxillary overdenture were included and randomly assigned to the group with four or six implants in the anterior region of the upper jaw. The implant and overdenture survival, clinical parameters (pockets, plaque, gingiva, bleeding and calculus), bone loss and patient satisfaction were assessed after 1 year, 5 years and in current studies after 10 years. Results: Thirty-one patients were evaluated after 10 years. A number of patients passed away, were unable to come or will come later in the year for evaluation. The survival of the implants was 100% and 98,7% respectively in the group with four implants and the group with six implants, in which 1 implant was lost. Both groups of patients scored well on the satisfaction questionnaire after 10 years and also the clinical functioning was good with no significant differences in the different aspects of the satisfaction questionnaire and clinical parameters between the groups (p>0,05). The average bone loss after 10 years was 0,40 ± 0,38 mm in the group with four implants and 0,35 ± 0,41 in the group with six implants. No significant difference was found between the groups (p>0,05). However, there is a significant difference in bone loss between the results after 1 and 5 years and between the results after 1 and 10 years (p<0,05). In the four-implants-group 8 maxillary overdentures have been replaced and in the six-implants-group 9 have been replaced Conclusion: An implant supported maxillary overdenture offers a good solution in the long term when it is no longer possible to obtain enough retention and stability with a conventional denture. This applies to both four and six implants in the anterior region of the upper jaw. The survival of the implants was high, patients are very satisfied and the clinical functioning was good. The average bone loss in both groups was minimal.
