Abstract

Introduction: At this moment an individualized treatment of OSAS cannot be delivered due to a lack of knowledge about the association between individual factors and treatment outcome of MAD therapy. When this is known, the practitioner will be able to estimate which patients will have beneficial effect of the MAD therapy. This is intended to make the therapy more efficient. The aim of this study was to determine the individual factors which will lead to a successful therapy. This study will focus on the question: “What is the association between different, individual factors on the treatment outcome of MAD therapy in patients with moderate to severe OSAS in the province of Groningen?” Material and Methods: The database of MZCW was searched for patients who have started with MAD therapy between 01-01-2013 and 31-06-2018. Inclusion criteria were the presence of both baseline registration and reregistration of the sleep examination in the database of OZG and a baseline registration of AHI ≥15. Primary outcome was success of MAD therapy based on AHI. Secondary outcomes were mean oxygen saturation, lowest oxygen saturation, ODI, snoring, heart rate, AHI in supine position and AHI in non-supine position. The individual factors and outcome variables were collected from the files of MZCW and OZG. The information was analyzed by doing a single and multiple logistic regression analysis for the primary outcome and a single and multiple linear regression analysis for the secondary outcome. Results: Out of a total of 612 patients, 191 patients were included in this study. Multiple logistic regression showed a significant (p < .001) relation between BMI and success of MAD therapy. Multiple linear regression showed a significant relation between BMI, age, smoking, daily alcohol use, medication and hypertension (p < .05) and one or more secondary outcomes. Discussion: This study shows that a high BMI and smoking are the most relevant factors associated with success of MAD therapy. Losing weight and quit smoking will lead to a higher success rate of MAD therapy. Further, according to the results dentists should consider offering alternative treatments to patients that are (morbidly) obese. The strength of this study is its large sample size (n = 191). However, the reliability of the results might be limited because this is a retrospective study.
