Abstract

Introduction: In the Netherlands, (23%) of the population smokes. Smoking has a negative impact on oral health. Because dentists and dental hygienists see their patients regularly, they can advise their patients to stop smoking. However, they are not always prepared to do so. The following question is central to current research: What is the attitude (described by means of the three-component model) of future and teaching oral care providers with regard to advising patients to stop smoking? Material and Methods: The research is an exploratory survey based on a (composite) questionnaire that measures the three components (cognitive, affective and conative) of attitude. 47 Master students (year 2 and 3), 40 oral care students (year 3 and 4) and 47 lecturers at the Centre for Dentistry and Monthly Care of the University Medical Centre Groningen participated in the study. The exploratory factor analysis showed that the cognitive component of attitude, as measured by the survey, could be divided into four parts: training and coaching, nicotine substitutes, medication and knowledge of smoking effects. The affective component could be divided into positive and negative emotions. The conative component could not be further subdivided. Results: From the results concerning the cognitive component, it was found that half of the participants were familiar with the tools of training and coaching to stop smoking, two-thirds were familiar with nicotine substitutes, participants had relatively little knowledge of smoking cessation medication, but had relatively much knowledge about the consequences of smoking. Both students and teachers indicated that they experienced predominantly negative emotions when giving advice to quit smoking. However, participants were generally willing to give advice to patients (conatieve component). Teachers had more knowledge about nicotine substitutes than students, but less knowledge about the consequences of smoking. Teachers scored higher on perception of advice, and were more hesitant about giving advice on how to quit smoking. Knowledge about the consequences of smoking and knowledge about training and coaching aides were found to be positive and significantly related to scores on the conative questionnaire. Conclusion: The students and teachers of dentistry and oral health care have no overwhelming willingness to advise to stop smoking. They have the knowledge about the consequences of smoking, but more attention should be paid to increasing the self-confidence of dentists in giving advice.
