Abstract

Effect of consensus instruction on interprofessional task division between dentistry and dental hygiene students Introduction: Interprofessional collaboration is advised to improve the organization of Dutch oral health. Perceptions on interprofessional collaboration are formed during interprofessional education. It is unclear to what extent interprofessional education can promote task shifting between dentists and dental hygienists. Objective: The aim of this study is to investigate whether consensus instruction leads to a more proportional division of tasks and can therefore promote interprofessional cooperation between dentistry and dental hygiene students. Main question: What is the effect of consensus instruction within a mixed profession group on interprofessional task division between dentistry and oral care students? Method: A double-blind randomized control group study is conducted. Third-year dentistry students and second-year dental hygiene students participated in the study. Mixed profession groups were randomly formed. Two experimental conditions were randomly formed: control-and intervention condition. All groups were instructed to elaborate patient casuistry. However, the intervention condition did this by implementing the consensus instruction. Examined was if a change occurred in the assignment of the following clusters to dental hygienists or dentists after consensus instruction: ‘General dental screening and diagnostics’ (cluster A), ‘Treatment of cariës’ (cluster B) and ‘Conventional dental hygiene treatments’ (cluster B). Results: The factor structure was in line with the expected clustering of tasks. The explained variance for each cluster was between 73% and 86%. Also, the internal consistency for cluster A, B and C was respectively α = 0.80, α = 0.87, α = 0.89. Intervention groups assigned cluster A tasks 1.9 times more to the dental hygienist in comparison with the control group. A medium-large effect was found for cluster A (Hedge’s g = 0.77). Intervention groups assigned cluster C tasks 4.4 times more to dentists in comparison with the control group. A borderline significance (p=0.055) and a large effect (Hedges’ g= 0.95) were found. Conclusion: This study has shown that consensus instruction can promote interprofessional division of tasks between dentistry and dental hygiene students with respect to two clusters. After consensus instruction, treatments such as screening and diagnostics are more often performed by dental hygiene students. Conventional dental hygienist treatments are more often performed by dentistry students after consensus instruction. Experimental effects could not be determined based on the significance level (α <0.05). However when using the effect size respectively medium large and large effects were found.
