Background: Cleft lip and palate is a congenital disorder which often occurs with dental anomalies such as absence of maxillary frontal teeth. This can cause functional and aesthetic problems that can be addressed in different ways. One of the treatment modalities is replacement by an implant supported crown. In non-cleft patients, previous research has been done on aesthetic and clinical outcome of dental implants in the aesthetic region. These studies revealed that pre-implant augmentation is associated with less favorable aesthetics of the surrounding mucosa. In cleft lip and palate patients pre-implant augmentation is always applied. Until now little is known about the aesthetic and clinical outcome of dental implant supported single tooth replacement in the cleft area. Objective: The objective of this study is to evaluate the aesthetic and clinical outcome of single tooth implants in cleft lip and palate region compared to single tooth implants in the maxillary anterior region in non-cleft lip and palate patients. Materials and methods: Nineteen cleft lip and palate patients participated in this study, 9 patients were seen a year after crown placement on the dental implant and 10 other patients were seen after 5 years. Their implant-supported single tooth replacement was scored by an experienced Maxillofacial Prosthodontist using the PES/WES index. Probing depth, bleeding on probing, Plaque index, Gingiva index, Papilla index and peri-implant marginal bone level change were evaluated as well. Each cleft patient was matched with a control non-cleft patient. Patient satisfaction was assessed using a short non-invasive questionnaire. Wilcoxon signed rank tests were used to analyze the data. Results: Between the 1 year cleft and the control group significant differences were found for Plaque and Gingiva index, both were less favorable in the cleft group. At 5 years the cleft group scored significantly less favorable on the PES score, Plaque index, Gingiva index, Papilla index on the distal side and Bleeding index. Furthermore, a significant difference was found in Patient satisfaction on form and color of mucosa and general satisfaction, with lower scores in the cleft group. Conclusion: The results of this study imply significantly less favorable outcome in clinical and esthetic parameters considering the mucosa surrounding the implant and crown in cleft and palate patients compared to the control group, especially after 5 years of follow-up.
