Abstract

Introduction. Dental wear is a multi-factorial condition and is becoming more common these days. This process results in a decrease of vertical dimension in front of the dentition. The fronts will grow past each other in a class-II (jaw) relationship without a stable occlusal contact. A deep bite or a palatum bite develops. The treatment of this condition in adult patients consists of surgery in combination with orthodontics. However, a removable Dahl plateau (also known as Dahlframe), in combination with active orthodontic treatment with fixed equipment (brackets), is also one of the possible treatment options. In the literature is little research on the dental changes of this combination treatment, which has been applied successfully in an orthodontic clinic for several years. This pilot study will map out the dental changes of this combination treatment. The average treatment duration is also examined and the outcomes will be compared with the traditional treatment of a deep bite. Research Material and Method. A total of 24 patients (older than 18 year) were included and underwent this combination therapy. Lateral skull photos (RSPs) were taken both before- and after the treatment. These RSPs were traced and superponated prior to the analyses. The variables, that maps out the dental changes, consists of vertical- and sagittal overbite (VOB and SOB), curve of Spee (CVS), height of the lower face half, inclination of the upper- and lower fronts and the degree of intrusion of the lower front. The average treatment duration is taken into account. The outcomes of all these variables will be compared with data from the literature of traditional treatment. Outcome. The use of a removable Dahl plateau, in combination with fixed equipment (brackets), has shown a significant difference for the VOB [t(23)=0,97;p=0,343],the CVS [t(23)=3,23;p=0,004], the height of the lower face half[t(23)=-3,71;p=0,001] and the inclination of the front elements [upper front: t(23)=- 3,44;p=0,002 and lower front: t(23)=-3,27;p=0,003]. The lower front is, on average, intruded with 0,50 mm and has a positive correlation with the VOB [F(1,22)=34,31;p=0,01] and CVS [F(1,22)=67,35;p=0,000]. It was found that 49,9% of the variability in the decrease of vertical overbite can be traced back to the levelling of the CVS. The average duration of this combination therapy is 547 days and is considerably shorter than the conventional treatment. Conclusion. A removable Dahl plateau, in combination with an orthodontic treatment with fixed equipment (brackets), is a smart application when you are treating a deep bite. The dento-alveolar complex can be extruded in the lateral parts so that the curve is flattened. This way the VOB will be less deep. Treatment results are comparable to traditional treatment, but the treatment duration is several months shorter. This combination therapy is a good alternative when surgery isn’t an option.
