Abstract

Aim A short implant can be a good solution in a resorbed jaw. However, in this situation it is often necessary to make a crown of considerable length to reach occlusion. This results in a higher crown-implant ratio (C/I-ratio) than the crown-rooth ratio with natural teeth where this number often lies between 0.5 and 1.0. It is assumed that a larger C/I�ratio will have a worse prognosis. The purpose of the research is to get more insight in the influence of C/I-ratio on peri-implant bone level and technical complications. The subgoal of the research is to get more insight in other factors of influence on the peri�implant bone level. Materials and methods For this research the peri-implant bone change of 220 short implants (8.5 mm) has been measured in 110 patient. At various evaluation periods, intraoral radiographs were made: immediately after implant placement(0 months afther implantation = T0m), immediately after crown placement(T5m) and 5 years thereafter(T5j). The clinical crown height was determined with the help of a computer program on the 5-years radiograph. The medical file was consulted for technical complications during 5 years of follow-up. C/I-ratio has been tested for possible relation with bone change and technical failures. Furthermore, several other subsets, such as the lower and upper jaw, gender and age, were compared using the Mann Whitney U test. Results The research failed to find a significant (p<0.05) relation between C/I-ratio and the amount of bone change. The average C/I-ratio in the research was 1.34 (±0.27) and the measured bone change was +0.07 (0.52) mm. Neither did the research find a significant correlation between the C/I-ratio and a group with technical complications and a group without technical complications. The research did find a significant difference between bone change at bridged implants in comparison to non-bridged implants, whereby bridged implants show more bone loss. Comparable results were found at implants with DAE+CaP surface. The research also found a significant difference in C/I-ratio between implants last in position and implants between other teeth. C/I-ratio is also significant larger in the group of implants placed in patients older than 50 years in comparison to implants placed in patients of 50 and younger. Conclusion Research showed that there is no influence of the C/I-ratio on peri-implant bone level changes and technical complications with 8.5 mm implants and crowns after five years. Implants of 8,5mm can be used safely. Bridged implants show unfavorable prediction in survival compared to non-bridged implants.
