Objective: The aim of this study was to evaluate the aesthetic outcome of maxillary anterior single implant crowns on bone-level implants in patients treated within the CTM, objectively by using the PES and the WES and subjectively by using the VAS. Research into this is important in order to identify and discuss any treatment difficulties, so that the quality of the treatment might be improved. Materials and methods: Seventeen patients, treated with maxillary anterior single implant crowns between 2011 and 2016, were enrolled in this study. To evaluate the aesthetics of the implant crowns, light photos were taken. Ten different dental specialties (two implantologists, two periodontists, two prosthodontists, two dentistry teachers and two dentistry students) evaluated the photographs using the PES and the WES. Patient satisfaction was assessed according to the VAS. Results: The mean total PES (4.99 ± 2.24) and WES (5.03 ± 1.62) both were less than the critical value of 6. Among the PES parameters, the mesial papilla showed the highest rating (1.2 ± 0.61). From the WES, the surface texture scored highest of the variables (1.31 ± 0.39). The mean patient satisfaction of the soft tissue aesthetics was 80.94 ± 19.65, of the white aesthetics 80.94 ± 19.65. Regarding the overall aesthetics, patients gave a mean satisfaction score of 91.06 ± 11.50. Implantologists were found to give the highest average score for the PES (5,74), prosthodontists the poorest (4,47). The difference between these groups was significant. Concerning this inter-rater reliability, no significant difference was found for the WES. Conclusion: This study demonstrated that the aesthetic outcome of anterior single implant crowns is insufficient for patients treated within the CTM. The degree of specialization did not affect the outcome. Tooth form in particular was identified to be an important aspect to focus on in order to improve the WES score. Regarding the PES, there are no specific parameters with significant lower scores than others. Nevertheless, patients are very satisfied with their tooth replacement. Data about the complexity of the cases, the patient related factors and surgical related factors, are not included in this study.
