Abstract

Introduction: Periodontal diseases are common. They are primarily linked to the presence of bacteria in dental plaque and calculus. In the treatment of these diseases, the primary focus is to increase the patients oral hygiene, combined with professional removal of plaque and calculus Objectives: The objective of this study is to acquire an insight into the effectiveness of therapy focussing on the prevention of periodontal disease as conducted by students within several study programs at the Centre for Dentistry and Dental Hygiene (CTM) at the University Medical Centre Groningen. Results: In total, 250 patient histories of patients who received at least two combined professional teeth cleanings and dental hygiene instructions between September 2015 and September 2016 were examined retrospectively up until 2012. This made it possible to extract information from 1267 treatment sessions. On average, a patient received 5.1 treatments (SD=2.4) with a total treatment span of 82.3 weeks (SD=61.1). In 73.2% of the investigated cases referral was (assumed to be) correct. 9.6% of verifiable referrals for preventive periodontal therapy were (presumably) false and 17.2% of referrals were not verifiable. No difference was found in plaque- or bleeding scores with increase of the number of treatments. Plaquescores were noted in 77.8% of all investigated cases. Bleeding scores in 72.0%. The Dutch Periodontal Screening Index (DPSI) was noted in 19.4% and the self-performed care regime was noted in 43.3%. In 12.2%, the patients smoking habits were noted. The given advice in 81.4%. The medical statusupdate was noted in 83.0%, the requested return interval in 59.7%. In the clinical programme Paro-1, plaque-, bleedingscores and DPSI were noted significantly more often than elsewhere. The retour interval was noted less often. General Practice Dental Hygiene noted the retour interval more often, Diagnostisch Centrum the DPSI more often and plaque- and bleedingscores less often. Staff noted less plaque- and bleedingscores and advices. General Practice Master noted less plaque-, bleedingscores and retour intervals. For self-performed care regime and medical statusupdate, no differences in frequency of notation were found between the different study programs. Conclusions: From this study it can be concluded that, based on 250 patients’ histories from 2012 up until 2016, 9.6% of verifiable referrals for preventive periodontal therapy were (presumably) false. 17.2% of referrals were not verifiable. The current regime does not give a reduction in plaque- or bleeding scores with increase of the amount of treatment sessions. Therefore, the programme has no effect. Relevant patient information is not being noted consistently in any of the investigated programs. A format text for noting relevant patient information in the patient statuses may increase the frequency in which the different information categories are noted, making way for increased effectiveness and efficiency in periodontal preventive therapy in the future.
