Abstract

Background The medical treatment for COPD and asthma plays a prominent role in the management of these obstructive pulmonary lung diseases and consists of inhalation medication. A manageable number of thoughtfully chosen devices in the shape of a formulary, which Curacao lacks, is the foundation to work uniform and guideline conform. Goal The goal of this research is to investigate whether the implementation of a formulary leads to individual health benefits. In addition, it is investigated whether the formulary can be implemented in a (medical) cost-neutral manner, and to what extent the implementation is deemed to be successful. Material and methods This research consists of a cohort study of 20 weeks (May 2019-September 2019). Hereby is the implementation of the formulary the intervention. The (MCID) scores of the ACT and CAT questionnaire, which were obtained by interviewing patients at GP practices, served as outcomes for individual health benefits. Medical costs were examined using data from the SVB. The degree of success of implementation was measured by questionnaires completed by GP’s (FIM/IAM/AIM). Results The research design consisted of a randomized sample of 50 people (control group n=25 and intervention group (n=25). There was no significant difference between the improving MCID’s when comparing the control group and the intervention group (32% vs 52%, difference 20%, p=0,152). Over a period of 5 months the medical costs for the intervention group remained approximately the same (70 ANG). The extent to which the implementation is successful, seems high with a high agreement for feasibility, appropriateness and acceptability. Conclusion The implementation of a new formulary doesn’t seem to lead to individual health benefits in the short term, however it could be implemented in a (medical) cost-neutral manner and the success of implementation is considered to be high.
