Abstract

Up until now, every Dutch medicine faculty uses their own tools to evaluate the educational environment during clerkships. None of these tools are based on a theoretical framework, and they also have not been validated for the Dutch clerkship context. Previous studies have shown that residents feel the need for a uniform compact survey, based on theory, to evaluate the quality of clerkships. Therefore we aimed to develop such an instrument: the concept ‘’SPEED” for clerkship students. The theoretical framework that has been used is characterized by three essential domains for a learning environment: content, atmosphere and organization. All of the items from currently used clerkship quality assessment instruments, combined with additional items from three instruments from the literature were collected and allocated to one of the three learning environment domains. The resulting base survey of 249 items was presented to a Delphi panel of five medical specialists, four residents and three educationalists. In three Delphi rounds, they were asked to select the items which they found to be the most relevant for the domain in question, and to ensure that the chosen items comprehensively covered the domain. The resulting draft “SPEED for clerkship students” contains 48 items: 16 in the content domain, 16 in the atmosphere domain and 16 in the organization domain. With this procedure, we have developed a well balanced draft tool to assess the quality of clerkships, which will be validated in a future study.
