Abstract

Objective: To optimize an aerobic cycle ergometer program, as part of the rehabilitation of patients with stroke, prior to full-scale implementation by exploring 1) the feasibility and related implementation components of the program; 2) the predictors for successful implementation; and 3) the preliminary effectiveness of the program. Design: Multiple-case design. Setting: Rehabilitation center. Participants: Three patients with ischemic stroke and mild motor impairments. Intervention: Supervised aerobic cycle ergometer program, 3 x/week for 8 weeks starting with 20 minutes exercise at 40% HRR up to 30 minutes at 70% HRR. Outcome Measures: Feasibility/Implementation components: program reach, adherence, dosage, quality, participant responsiveness, program differentiation, comparison conditions, adaptations. Predictors: provider and innovation characteristics, factors related to program delivery and support system. Effectiveness: exercise capacity (SLBE test), functional performance (6-MWT). Results: Feasibility/Implementation components: Three of 12 patients were included (1 female, 2 males, mean age: 65.7 ± 5.1 years). Attendance rate was 66.7-100%. Patients trained 2-3 x/week, 20-30 min/session at 40-85%HRR without adverse events. Quality was excellent (8.8), participant responsiveness and program differentiation were both sufficient to excellent (7 .0-8. 7 and 7.2-8.5, respectively), usual care consisted of 2-4 other therapies, adaptations were made in training protocol regarding all aspects (frequency, duration, intensity). Predictors: Moderate to sufficient scores were given for provider characteristics (before program: 5.7-6.3; after program: 6.7-7.7). Sufficient scores were given for innovation characteristics (7.7). Regarding factors related to program delivery, several logistical problems occurred. The support system were judged with sufficient to excellent scores (7 .2-8. 7). Effectiveness: Improvements were found in aerobic capacity (11-20%), power output (22-50%), motor efficiency (9-23%), anaerobic threshold (8-300%), and walking distance (6-15%). Conclusion: Despite the low program reach, the aerobic cycle ergometer program was I) feasible, safe, well delivered, and well tolerated in the rehabilitation of patients with stroke when adaptations in training protocol were made; 2) compatible, adaptable, and well supported, but more attention is needed for providers and logistical problems; 3) effective for improving exercise capacity and functional performance.
