Abstract

Objective: Vestibular schwannoma is a benign cerebellopontine tumor that has been associated with a reduction in quality of life. An invalidating complication of this tumor is a facial paralysis. The aim of this study was to evaluate the influence of a facial paralysis on the quality of life, and the patient’s emotional and social behavior. Design: Case control pilot study Method: Twelve patients were included with a facial paralysis, of which five were caused by a vestibular schwannoma and seven were caused by a different aetiology. The study group was compared to a compatible, healthy control group. All participants filled out a set of six validated questionnaires and additional questions about the facial paralysis. These questionnaires have been used to measure the quality of life, emotional and social behavior. Participants’ facial paralyses were graded using the House Brackmann Grading Scale. Results: The entire study group was compared to the healthy control group. Significant differences were found on all questionnaires, except one, indicating that the study group with a facial paralysis perceives a worse quality of life compared to the healthy control group. These scores also indicated that the study group perceived more social behavioral problems and showed more signs of a depression compared to the healthy control group. In mutual comparison of the patients with a vestibular schwannoma to the patients with a different aetiology of their facial paralysis, no differences were found in answering the questionnaires, only in answering the amount of burden experienced, caused by having a facial paralysis. The patients with a vestibular schwannoma experience significant more problems caused by a facial paralysis. Conclusion: Patients with a facial paralysis showed a significant decrease in quality of life and show more emotional, social and behavioral problems in comparison to a healthy control group. Although the sample size was small, significant differences were found, indicating that patients with a facial paralysis perceive a lower quality in life, more social behavioral problems and have more characteristics of a depression. Further investigation is needed to make the results more reliable and proof that they are also applicable to a larger group.
