Abstract

Background: Arterial calcification is a common marker for atherosclerosis, intracranial arterial calcification is an independent risk factor for ischemic stroke. For coronary artery calcification, the most studied form of arterial calcification, multiple genetic and cardiovascular risk factors are identified, but their relation to intracranial arterial calcification, specifically hippocampal calcification, isn’t studied extensively. Methods: In two patient cohorts with a total of 1239 patients CT scans are scored on the presence and severity of hippocampal calcification. 12 risk factors and 8 single nucleotide polymorphisms (SNPs) are extracted to define an univariate and multivariate odds ratio (OR) with 95% confidence interval (CI) for each risk factor on presence of calcification, and moderate-severe calcification. After this a meta-analysis was conducted. Results: In the first cohort age≥60yr (OR (95%CI): 2.47 (1.37-4.45)) and vitamin K-antagonist use (3.13 (1.04-9.43)) are significant risk factors for presence of hippocampal calcification. In the second cohort age is a risk factor for calcification presence (1.91 (1.30-2.82)), age (2.77 (1.36-3.65)) and male sex (2.11 (1.00-4.46)) for moderate-severe calcification. In meta-analysis age≥60 is significant for presence (2.10 (1.53-2.90)) as well as moderate-severe calcification (2.26 (1.30-3.90)). Smoking, hypertension, and cumulative risk didn’t show any association. Diabetes, kidney failure, patient history, and all genetic risk factors didn’t have enough power to draw conclusions. Conclusion: Higher age is a clear risk factor for hippocampal calcification. In this study, male sex, smoking, and hypertension aren’t related to hippocampal calcification showing that risk factors differ between intracranial calcification and other types of arterial calcification. More extended research on intracranial calcification, on different locations and risk factors, is needed.
