Abstract

Introduction: Recent research shows that the endometrium of patients with Asherman’s syndrome persists in the luteal phase (the ‘endometrial secretory arrest’). Because uterine peristalsis (UP) is becoming quiescent in the same part of the cycle, it is expected that UP is not present with this disease. This pilot study tries to provide insight in the course of UP in patients with Asherman’s syndrome on video images made with transvaginal ultrasound (TVUS). Material and method: Video images of 1 minute in the mid-sagittal plan of the uterus are recorded with TVUS in 15 patients with Asherman’s syndrome. These images are judged using a motion analysis and afterwards interpreted on a characteristic pattern of UP. Results: No movement is observed in a total of 10 patients (67%). In 5 patients (27%) some degree of movement is seen, while the uterus of only 1 patient (7%) shows actual shortening and extension of the sagittal axis. However, all movements found do not meet the characteristic pattern of UP. As a result UP is not observed with any of the patients. Conclusion: The results of this pilot study show that UP is not observed in patients with Asherman’s syndrome. Therewith there seems to be a dysfunction of the contractility of the myometrium simultaneously with a dysfunction of the endometrium (the ‘endo-myometrial arrest’).
