Abstract

Introduction. About 15% of the adult population visiting the preoperative screening is diagnosed with obesity. About 35% of these patients silently suffer from metabolic syndrome and are at risk to develop diabetes and/or cardiovascular disease. Furthermore, patients may be at risk to develop perioperative hyperglycemia. At this moment a trial (the POSitive II study) is performed in the VUmc Amsterdam, where patients are screened for impaired glucose tolerance by measuring HbA1c point-of-care. However, this analyzer has not been validated yet for this setting and population. Also it is uncertain which patients are at risk for having a high HbA1c. Methods. In this prospective observational study we included 90 patients (who were screened for the POSitive II study) with a body mass index (BMI) > 25 kg/m2 and without history of diabetes. Of 49 patients blood samples were analyzed both by the Siemens DCA Vantage point-of-care HbA1c analyzer and in the laboratory to study the level of agreement between both methods. Secondary outcomes were associations and predictive values between BMI, waist circumference, age and HbA1c were evaluated for both men and women. Results. The Bland-Altman analysis calculated a level of agreement from -0.39% to 0.28% HbA1c. The coefficient of variation was found to be 3.02%. Statistically significant correlations were found for age, waist circumference and BMI in men (r=0.31, 0.49, 0.38 respectively). In women no significant correlations were found (r=0.08, 0.04, 0.12 for age, waist circumference and BMI respectively). Conclusion. The Siemens DCA Vantage HbA1c analyzer has a high level of agreement to the laboratory test and can therefore be used as a screening instrument at the preoperative screening. In contrast to women, waist circumference was found to be an independent predictor of HbA1c in men.
