Abstract

Introduction: Emergency ultrasound is a new imaging technique that is used by emergency physicians in the emergency department (ED) to answer a simple question which is important for the diagnostic process and treatment. There are many different indications and protocols for emergency ultrasound including the rapid ultrasound in shock (RUSH) protocol and the focused assessment with sonography in trauma (FAST). Emergency ultrasound is becoming an increasingly popular diagnostic tool in the ED and is seen as an extension of the physical examination. Goals: What is the effectiveness of emergency ultrasound and what are the effects on determining the diagnosis and the therapeutic and diagnostic treatment plan. Methods: In a prospective study at the emergency department in the UMCG, the effect of emergency ultrasound on the diagnosis, the certainty of the diagnosis, the number of items in the differential diagnosis and the effect on the treatment was investigated. To evaluate the effect of the experience of the doctor the amount of previous made emergency ultrasounds was recorded. Results: Pulmonary ultrasound was with 29% the most preformed indication for emergency ultrasound. In 24% of the cases there was a change in diagnosis, and within this group significantly more changes in the treatment plan occurred. In 39% of the cases emergency ultrasound resulted in a change of treatment plan. The doctor was significantly more confident of his diagnosis after making an emergency ultrasound (p<0.05). Additionally there where less diagnoses in the differential diagnosis after making an emergency ultrasound (p<0.05). The higher the level of experience was, the bigger the difference in certainty after making an emergency ultrasound (p<0.05). Conclusion: Using emergency ultrasound leads to fewer items in the differential diagnosis, changes in working diagnosis and more certainty about the diagnosis. Besides, performing emergency ultrasound results to changes in diagnostic or therapeutic treatment plan.
