Abstract

Background: Elderly patients often suffer from deteriorated cognitive and physical functions during acute hospitalisation and are prone to adverse outcomes. In St.-Josefs-Hospital, Cloppenburg acute hospitalised geriatric patients receive Geriatric Early Rehabilitation Complex Treatment (GERCT) on a geriatric ward to prevent them from these adverse outcomes. Objectives: To analyse the effects of GERCT in St.-Josefs-Hospital mainly concentrated on patients’ discharge destinations. Successful intervention is considered as the direct return to the own home after discharge. Methods: This study is a retrospective evaluation of all geriatric patients who were discharged between January 1 and December 31, 2016 and who received GERCT for at least 14 days. Particularly the frequency of the discharge destinations of the community-dwelling patients were examined due to their higher likelihood of a direct return to their own home. Results: 197 acute hospitalised geriatrtic patients were discharged in the examined period. We particularly analysed the cases of 171 community-dwelling acute hospitalised geriatric patients for the direct return to the own home. The study cohort of those community-dwelling patients consists of 110 female and 61 male patients. Their mean age was 81.8 years (± 5.9). Of the (n=171) community-dwelling patients, 49.7 % could directly return to their own home after discharge, 24.5 % were sent to a rehab-clinic, 19.9 % were sent to intermediate care in a nursing home and 1.2 % were referred to long-term care in a nursing home after discharge. Conclusion: One half of the acutely hospitalised community-dwelling patients could return directly to their own home after hospitalisation. The selection of the patients for GERCT and the comprehensive treatment in St.-Josefs-Hospital therefore seems to be successful. Patients of each examined discharge destinations have characteristic features. Age (p < 0.001) and the results of the Geriatric Assessments such as good MMSE, good Barthel-Index and good Tinetti-Test ( all p < 0.001) at discharge seem to be predicting factors for the direct return to the own home after acute hospitalisation.
