Abstract
Aim of the study The primary aim of this retrospective study was to evaluate certain surgical, orthodontic and dental aspects after bone grafting of the alveolar cleft on patients with cleft lip and palate. A second aim was to study the relation between these aspects and the type of cleft that was diagnosed before treatment. Method Data were collected of patients who were diagnosed with a cleft alveolus together with a cleft lip and/or palate and were treated with bone graft transplantation between January 1st, 2005 and December 31th, 2010 in the Department of Oral- and Maxillofacial Surgery, Maxillofacial Prosthetics and Orthodontics of the University Medical Centre Groningen (UMCG). 58 patients were included in this study. The collected variables were processed in a database in SPSS. These results were analyzed using a Chi-square test, Fischer's exact test and Monte Carlo test. For all statistical tests a significance level of0,05 was used. Results In 66,7% of the cases a cleft lip and alveolus (CLA) occurs in female patients. A cleft lip, alveolus and palate (CLAP) occurs in 65,1% of the cases in male patients. These results were found significant with the Chi-square test. A tendency to a relation between CLAP and dehiscence of the bone graft is found, this relation is not significant (p=0.097 2-sided). The most frequent orthodontic aspect in a CLAP was the phenomenon that the cuspid did not erupt spontaneously after bone graft transplantation. In a CLA this occurs in 40% of the cases and in a CLAP in 27,9% of the cases. A tendency to a relationship between a CLA and supernumerary teeth (p=0.091 2-sided) was found in comparison with a CLAP. A significant relation is found between a CLA and tooth form deviations (p=0.017 2-sided) in comparison with a CLAP. The results were found with the Fischer's exact test. A further common dental aspect is the agenesis of the lateral incisor. Conclusion The most common orthodontic aspect found after bone grafting of the alveolar cleft is the fact that the cuspid does not erupt spontaneously. This necessitates surgical exposure, before orthodontic traction can be performed. Agenesis of the lateral incisor and supernumerary teeth were other frequent dental observation in this study. Wound dehiscence is a complication that has a tendency to a significant relationship with a CLAP and is the most frequently found surgical complication after bone grafting of the alveolar cleft. Tooth form deviations in close proximity to the cleft have also a significant relationship with CLA.
