Abstract

Purpose: The aim of the study is to determine if the Dutch translation of the Achilles-tendon-Total-Rupture-Score (ATRS-NL) is valid and reliable to evaluate symptoms and physical functioning in Achilles tendon rupture patients. Background: The ATRS was developed in Sweden (2007), because of the need for a valid, reliable and sensitive PROM to evaluate symptoms and physical functioning in Achilles tendon rupture patients. The ATRS questionnaire is already translated in Danish, Turkish and English. Recently, it is also translated into Dutch. However, before a questionnaire can be used in a clinical setting, it is necessary to determine the validity and reliability. Methods: Of the 160 patients who were treated for Achilles tendon rupture in the UMCG between 2010 and 2014, 93 participated in the validity study and 54 in the reliability study. 18 patients participated in the echo study. For the validity study the ATRS-NL was compared to four questionnaires (VISA-A, FAOS, SF-36, VAS for pain). The test-retest reliability was determined by filling in the ATRS-NL for a second time after a period of two weeks. Cronbach’s alpha was used for intern consistency and the ICC for measuring the test-retest reliability. Results: The Dutch version of the ATRS showed moderate construct validity (60% of the a priori defined hypotheses were confirmed). The ATRS-NL showed high correlation with the VISA-A (r = 0,73), low to high correlation with the subscales of the FAOS (r = 0,26-0,76), low to moderate correlation with the subscales of the SF-36 (r = 0,07-0,68), low to moderate correlation with the subscales of the VAS for pain (r = 0,26-0,56) and low correlation with the echo (r = 0,10-0,12). The intern consistency (Crohnbach’s alpha= 0.96) and the test-retest reliability (ICC = 0.902) were excellent. The SEM was 4,48 and the MDC 12,41. Conclusion: The ATRS-NL has been proven moderate valid and good reliable to evaluate symptoms and physical functioning in Achilles tendon rupture patients.
