Abstract
Background: Literature about efficacy of radiotherapy (RT) for head and neck cutaneous squamous cell carcinoma (cSCC) is sparse. Previous studies are hard to compare due to wide variety in study designs and outcome measures. Objective: The main purpose is to determine the locoregional control rate of head and neck cSCC after RT. Furthermore, we determined whether specific patient, tumor and treatment characteristics may influence the outcome after RT. Methods: Retrospective cross-sectional database study. Patients with head and neck cSCC who were treated with RT between 01-01-2000 and 01-01-2014, met the inclusion criteria. Results: A total of 138 patients with 151 cSCC were included. The overall two- and five-years locoregional control rates were 77.6% and 71.4%, respectively. There was no significant difference in locoregional control rate between the three main groups (Log-rank P=0.545). In multivariate analyses tumor size more than 2 cm and poor differentiation grade were significantly independent predictive factors of local recurrence and metastases. Extradermal invasion (T3-stage) was significantly associated with higher risk of local recurrence, but not with metastasizing. Conclusion: Radiotherapy could be an effective treatment for patients with head and neck cSCC. However, outcome after RT may be less favorable for larger cSCC (> 2 cm), with extradermal invasion or a poor differentiation grade.
