Abstract

Background: Headache is one of the most common somatic complaints in children and may have a strong influence on daily functioning and quality of life. There is often no clear pathway for children with chronic headaches and therefore multidisciplinary headache centres seem to be a good development. To our knowledge however, few research has been done on this topic. Objective: The aim of this study is to evaluate the effectiveness and outcome of an outpatient multidisciplinary headache department for children with chronic headaches. A second objective is to provide insight into the prevalence of the different diagnoses and treatment policies in this clinic. Methods: A retrospective observational study for children between 5 and 16 years was performed at the multidisciplinary headache clinic of the Medisch Spectrum Twente in Enschede. Between November 2006 and November 2011 data were collected on headache characteristics prior to the initial visit and after 6 and 12 months. These characteristics included headache frequency, intensity and duration and frequency of medication use. In addition the diagnoses and treatment policies were analyzed. Results: The files and questionnaires of 154 children were evaluated. In 92,9% of the children a diagnosis was made. Chronic tension-type headache was the most frequent cause of headaches (28,6%). In 16.9% additional tests were performed and 76,6% received at least one type of treatment. Referral to a child psychologist was the most frequent treatment intervention (46,1%). A significant reduction in headache intensity and frequency as well as medication intake was seen between the first time point and 6 months and between the first time point and 12 months. Furthermore parents were mainly positive about the course of the headaches and the treatment applied. Conclusions: This outpatient multidisciplinary headache department seems to have a positive effect on chronic headaches in children between 5 and 16 years of age. The headache intensity and frequency as well as medication intake improved significantly.
