Abstract

Backgrounds: The multidisciplinary approach is becoming increasingly encouraged, also in oncology where a treatment for each patient is custom made. Aims: To evaluate treatment proposals by a multidisciplinary team (MDT) for patients with an urological malignancy, degree of agreement between treatment proposals from doctors and MDT’s was calculated. Besides extent to which the patient chose the treatment proposed by a MDT was explored. Methods: Patients with an urological malignancy of prostate, bladder, kidney or testicle who were discussed at the uro-oncology MDT in the ZiekenhuisGroep Twente from January 2011 until January 2013. After collecting MDT letters, this study looked how often treatment proposals from doctor and MDT matched completely, partly or not at all. By using Cohen’s Kappa, degree of agreement between treatment proposals from doctor and MDT could be established. Also treatments proposed by the MDT were compared to the final treatment choice the patient made. Results: A total of 788 letters was analysed. For 9%, the MDT disagreed with the treatment recommended by the doctor. Especially treatment proposals for patients with kidney carcinoma showed less degree of agreement. Over 90% of all patients elected to take (one of) the treatment proposals made by the MDT. Conclusion: Introduction of a MDT appeared of value in the development of a treatment proposal for patients with an urological malignancy. This multidisciplinary approach seemed particularly useful for patients with cancer of the kidney. However, a multidisciplinary team could be of great value for patients with prostate carcinoma too, because often several treatment strategies were possible. The ultimate treatment choice of patients with urological cancers suggests patients have confidence in the MDT’s.
