Abstract

Introduction: Primary, traumatic patellar dislocation is usually an acute event and may be followed by redislocations of the patella, which is called chronic patellar instability. Patients with chronic patellar instability have serious symptoms and disability, and have severe restriction of movement of the knee and limitations in activity levels of the patients. However, little is known about the differences between the medium-term results after primary patellar dislocation and chronic patellar instability. There may be prognostic factors for the development of chronic patellar instability. These factors can be either congenital or acquired. In this study differences between medium-term results in term of physical function and quality of life of patients with primary patellar dislocation and patients with chronic patellar instability are determined. This study also examined the incidence of redislocation of the patella after primary patellar dislocation and the prognostic factors for the development of chronic patellar instability. Methods: This study is a cohort study. The study was conducted at the Trauma Surgery Department of the University Medical Center Groningen. On the basis of a database of the past 15 years held in the UMCG, data are collected in the period between 01-01-1998 and 01-09-2008. Patients completed a general questionnaire and scoring lists (IKDC-NL, SMFA-NL and SARS), which have been converted for statistical analysis. For the statistical analysis a non-parametric analyze and logistic regression analyzes were used. Results and discussion: This study, between 95 patients, showed no significant differences in the medium-term results in terms of physical function and quality of life of patients with primary patellar dislocation and patients with chronic patellar instability. However, there seemed to be a decreasing trend to exist in the scores of the IKDC-NL and the SARS. While an increasing trend existed in the scores of the SMFA-NL. The incidence of a redislocation of the patella after a primary patellar dislocation is 56% and the incidence of chronic patellar instability is 34%. Prognostic factor for the development of redislocations of the patella, according to this study, is the age at the time of the primary patellar dislocation. Factors like age at the time of the primary patellar dislocation, if the dominant leg was affected, symptoms prior of the primary patellar dislocation, BMI at the time of the primary patellar dislocation, the cause the primary patellar dislocation, the treatment of the primary patellar dislocation, sport after primary patellar dislocation, symptoms after primary patellar dislocation and a positive family history of knee problems appeared predictive for chronic patellar instability. Only age at the time of the primary patellar dislocation appeared to significantly affect the emergence of a redislocation of the patella. Conclusion: this study describes that there are no significant differences detectable in the medium-term results in terms of physical function and quality of life of patients with primary patellar dislocation and patients with chronic patellar instability. The incidence of a redislocation of the patella after a primary patellar dislocation is 56% and the incidence of chronic patellar instability is 34%. Based on this study, age at primary patellar dislocation is a prognostic factor for the development of redislocations of the patella.
