Abstract

Purpose: The purpose of this pilot study is to determine if cross-education can reduce the loss of proprioception, static and dynamic balance in the operated leg, and accelerate return to work and sports after ACL reconstruction. Method: 4 subjects were randomly assigned either to the standard care or the standard care+ crosseducation group. The standard care group received standard physiotherapy after ACL reconstruction and the standard care+ cross-education group received after surgery besides the standard physiotherapy also strength training of the healthy leg. One week prior to and 5 weeks pre-surgery 3 tests were conducted to test for balance and proprioception; the one leg balance test, the STAR Excursion Balance Test and a repositioning task on a dynamometer. Results: In the standard care+ cross-education group the healthy leg improved performance on all three tests. In this group the operated leg improved only on the one leg balance test, eyes closed. A decrease in performance was seen on the other two tests. In the standard care group small improvements are seen on all tests for both legs. Discussion: The pilot study allows us to draw limited conclusions. Still, these preliminary data reveal some trends suggesting a potential role of cross-education in ACL rehab. The findings on the one leg balance are consistent with previous research. The results of the SEBT and the proprioception task differ from previous research. Follow-up measurements subjects should be taken to determine if the crosseducation effect is present 3 and 6 months after surgery. Conclusion: The limited data suggests that there is a positive effect of the cross-education training on static balance. Cross-education seems to have no effect on the proprioception or the dynamic balance after 5 weeks of physiotherapy.
