Abstract
Background: Worldwide, childhood obesity is still becoming more prevalent, with a growing prevalence among preschool-aged children. Studies have shown associations between the grade of overweight and the children’s eating style. Eating behaviors with a tendency to overeat emerge early in the developmental pathway and show individual continuity. Also, health-related quality of life (HRQoL) and childhood obesity seem to be associated. Obese children show lower scores for HRQoL, especially on scales for self-esteem and physical functioning. Multidisciplinary obesity treatment programs in school-aged children have demonstrated to improve HRQoL. Effects of multidisciplinary obesity treatment programs in preschool-aged children remain largely unknown. Objective: To evaluate the effect of a multidisciplinary intervention program in 3- to 5-year-old overweight and obese children on health perception and quality of life, and to assess the effects on eating behavior of both the children and their parents. Method: 75 overweight or obese children, aged 3- to 5-years were randomized to a multidisciplinary intervention program (including consultations of a dietician, a psychologist and a physiotherapist.) or to a usual-care program (including 3 consultations of a pediatrician). At baseline, after the end of the program (16 weeks) and 12 months, anthropometry and body composition was performed and parents completed the Dutch-Child-AZL-TNO-Quality-of-Life (DUX-25) questionnaire, the Child Health Questionnaire for Parents (CHQ-PF50) and the Child (DEBQ-C) and Dutch Eating Behaviour Questionnaire (DEBQ). Results: At 12 months follow-up, children in the multidisciplinary intervention group showed a greater increase in the total quality of life group (5.0 (-1.8;6.5) vs. -4.5 (-15.3;4.0)) (p=0.04), especially for the physical domain (8.3 (-6.3;16.7) vs. -4.2 (-12.5;4.7) (p=0.03), compared with children in the usual-care group. A decrease in several overweight- and obesity-parameters was associated with an increase in HRQoL. Children in the multidisciplinary intervention group showed a trend toward increased restrained eating behavior (main-effect p=0.07). Conclusion: A multidisciplinary intervention program in 3- to 5-year-old overweight or obese children has beneficial effects on HRQoL, compared with a usual-care program.
