Abstract
Analysis of the morphology of the medial wall and orbital floor in adults for the development of prefabricated implants. Objective: The aim of this study was to analyze the morphology of the orbita and explore the possibility of producing a prefabricated implant that can be used for the reconstruction of the medial wall and orbital floor in cases fracture. Method: 50 CT scans already on file (n=50; age >17; 25 males and 25 females) from patients undergoing diagnostic procedures nearby, but not directly involving the orbita (e.g. paranasal sinus problems) were randomly selected. Exclusion criteria were underlying conditions such as congenital craniofacial malformations, history of orbital fracture, malignancies in the orbital region or otherwise acquired deformations of the orbita. First the CT scans were first automatically segmented, then the orbitae were manually drawn to prevent the formation of pseudo foramina. Next, three different angles (∠PLC, ∠IOF and ∠OB/OR) were determined, where after a 3D model was made and transported to a second software program by the use of a STL+ module. With this software program the surface area SA of the medial wall and orbital floor were determined. The data was analyzed by calculating the means and standard deviations and performing an ANOVA. Results: The intra-observer reliability was tested and with an estimation error of 2,9% it is considered as acceptable. No significant differences were found for the angles ∠PLC (p = 0,209), ∠IOF (p = 0,230) and ∠OB/OR (p = 0,776) between the genders. For the surface area SA a gender dependent significant difference was found (p < 0,000). Conclusion: The individual variability for the determined angles (∠PLC, ∠IOF and ∠OB/OR), as well as the surface area has such a great scope, that it isn’t possible to produce a prefabricated implant. Further development of software is necessary to perform the segmentations automatically.
