Abstract

Purpose: Periodontitis is a destructive inflammation of the periodontium. Periodontitis is a multifactorial disease with an important bacterial component. Rheumatoid arthritis (RA) is a chronic destructive inflammatory disease of the synovial joints which affects the synovial membrane and joint structures. Currently the periodontal bacterium P. gingivalis is of high interest in the relationship between periodontitis and RA. This micro-organism has the unique feature to citrullinate proteins. Antibodies against citrullinated proteins (ACPA) are highly specific for RA. The purpose of this study was to compose a group of healthy controles, without periodontitis and RA, to determine negative references for the anti-P. gingivalis titers. Materials and Methods: 36 subject with age older than 18 years, a healthy periodontium (DPSI 0) or local gingivitis (DPSI 1 or 2 and <30% of measured locations showed bleeding on probing) were enrolled in this study. The DPSI score was measured, subgingival plaque samples and 10 ml peripheral blood were taken. The identification of the anaerobic subgingival microflora was done by standard anaerobic culture procedures. Measuring of the anti-P. gingivalis titers was done by the ELISA technique. The found values were compared with a group of RA patients (n=95) and a group of matched non-RA patients (n=44) from an ongoing study in the UMCG. Results: The mean anti-P. gingivalis titers of IgA, IgG and IgM in persons without periodontitis and no detectable subgingival P. gingivalis are 1.93±1.67, 1.71±1.70 and 11.5±8.74 mg/l, respectively. With severe periodontitis there are significantly higher IgA, IgG and IgM anti-P. gingivalis titers in RA patients than the negative references. Also, RA patients with severe periodontitis have significantly higher anti-P. gingivalis titers than patients with severe periodontitis without RA, except for the IgA anti-P. gingivalis. With moderate periodontitis there are only significantly higher IgA and IgG anti-P. gingivalis titers in non-RA controles than the negative references. Conclusion: Patients with RA and severe periodontitis have a more pronounced immune response against P. gingivalis than periodontal patients without RA.
