Abstract

Introduction An oroantral communication is defined as the space created between the maxillary sinus and the buccal cavity. The most common cause of an oroantral communication is the extraction of the maxillary posterior teeth. 1 ' 2 Literature review has not shown any conclusions about complication-ratio after surgical closure of oroantral communications. Purpose The University Medical Center Groningen has started a treatment in 2008 which is different from the conventional surgical procedure to close an oroantral communication. In this new treatment a polyurethane foam is placed in an extraction wound, after which the wound is closed primarily. To compare the conventional surgical procedure with the treatment in which the polyurethane foam is used, the complicationratio after surgical closure of oroantral communication in the period 2004 up until 2008 has been studied in this thesis. Materials and methods A retrospective study of 323 patient files has been conducted in the University Medical Center Groningen. A total of 355 closures of oroantral communications has been performed. These could be first time treatments, or retreatments of an oroantral communication. Logistic regression analysis was used to assess the relationship between predictive variables and patient outcome (retreatment). Conclusion The complication-ratio of surgical closure of oroantral communications, as shown in this retrospective study, is almost 10%. The complication-ratio of the treatment with the polyurethane foam has to be as much as or less than the complication-ratio of the surgical closure of oroantral communication, to be acknowledged as a suitable alternative for surgical closure.
