Abstract

Little is known about the relationship of walking speed and gait stability for people walking with a lower limb prosthesis. The aim of the study was therefore to investigate whether there is an effect of walking speed on selected measures of stability and how this compares between amputees and healthy controls. Twenty one able-bodied controls and twenty one amputees (13 transtibial, 8 transfemoral) walked at five different speeds varying from comfortable walking speed (CWS) to plus and minus 15 and 30% of CWS. Selected measures of stability were mean values and variability of step width (SW), step length amputation/sound (SLamp/sound), stride time (ST) and dual support time amputation/sound (DST amp/sound). Groups were compared on the selected measures of stability and a possible optimal walking speed was investigated. Results showed that SW, SLsound, DSTamp and SW variability can differentiate between controls and amputees. Further there is an effect of speed on all the selected measures of stability except for SW and SW variability. Only the relationship of speed and DSTamp is group dependent. At last, controls showed a minimum in stride time and step length variability just around CWS. This was not seen in the amputee groups for the speeds walked at. It can be concluded that amputees are less stable than controls and that stability depends on walking speed, but that the relationship between walking speed and stability is not group dependent. Further CWS for able bodied controls might be explained in terms of gait stability, while amputees select CWS independent of an optimum in any of the stability parameters. Therefore it could be wondered whether amputees choose an optimal walking pattern in terms of stability.
